24. The incidence of documented pulmonary embolism with "normal or near normal" scans in the PIOPED study was five of 131 patients. However, only 42% of those patients agreed to arteriography. In only 21 of the 131 patients did both radiologists interpret the scan as negative. In only three of these cases was arteriography obtained. Results were negative in all three. None of the remaining 18 patients had clinically apparent pulmonary embolism at the time of follow-up. Therefore, no patient whose V/Q scan was considered negative by two readers was subsequently proven to have pulmonary embolism. However, since arteriogaphy was not performed on 58% of the 131 patients with "normal or near normal" scans, the true incidence of false-negative "normal or near normal" V/Q scans cannot be determined. Since five of the 131 patients with "normal to near normal scans" had clinically documented pulmonary embolism, at least 4% of patients whose scans were read as normal by an experienced radiologist were subsequently shown to have had a pulmonary embolism. Thus, 4%, in our opinion, is the best estimate of the rate of error that the primary care physician might expect in community practice.
